  • I acknowledge that this Accident Waiver & Release of Liability (AWRL) form will be used by the organizers of the event, in which I participate & will govern my actions at said events. If volunteer is a minor, parents and guardians agree to the terms of this volunteer waiver. In consideration of my application & permitting me to volunteer at this event, I hereby take action for myself, & assigns as follows: (A) Waive, Release & Discharge from any & all liability for my death, disability, personal injury, property damage or theft, or actions of any kind which may hereafter accrue to me, THE FOLLOWING ENTITIES OR PERSONS: The Lily Pad Foundation & it's subsidiaries, Methodist Rehabilitation Center & it's subsidiaries, surrounding businesses, event sponsors, event permitters, event donors, their directors, employees & representatives, the event holder; (B) Indemnify & Hold Harmless the entities or persons mentioned in this paragraph from any & all liabilities or claims made as a result of participation in this event. This AWRL shall be construed broadly to provide a release arid waiver to the maximum extent permissible under applicable law. The volunteer agrees to the use of his/her name, photographs, videos, comments and any audio in broadcasts, newspapers, brochures and other media without compensation.
